Your initial appointment will consist of a comprehensive dental exam, gum tissue exam, TMJ exam, panoramic x-ray, intra-oral x-rays, and any photographs needed for proper diagnosis. Depending upon the complexity of dental treatment Dr. Rider is recommending, a complementary consultation appointment may be scheduled to review and explain all treatment options.
If you are experiencing dental pain, we will do our best to accommodate you the same day that you call. We will make every effort to relieve your pain through emergency treatment and then schedule you for definitive care.
Please assist us by providing the following information at the time of your appointment:
- Health History Form, Financial Policy Agreement, HIPPA Consent Form – completed prior to your arrival.
- Primary Care Physician name and phone number
- A list of medications you are presently taking
- Dental Insurance information
If you have dental insurance, please bring your current insurance card; however, if you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
Please alert our office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, osteoporosis, medicine allergies, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, bisphosphonates, etc.).
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian and have all forms completed and signed by a parent or guardian at the first visit.
